
Rehabilitation Therapy Services
Our Assisted Living Facility supports residents in maintaining and improving their strength, mobility, independence, and overall quality of life through coordinated rehabilitation therapy services. We work closely with residents, families, physicians, and licensed therapy professionals to help residents achieve their individual goals and maintain their highest level of function.
Rehabilitation therapy services may include:

Physical Therapy (PT) – Support with mobility, balance, strength, endurance, fall prevention, and safe movement as ordered by the resident’s healthcare provider.

Occupational Therapy (OT) – Assistance with improving independence in daily activities, adapting routines, and promoting safe participation in personal care and household tasks.

Speech Therapy (ST) – Support with communication, cognitive-linguistic skills, and swallowing needs when recommended by a licensed healthcare provider.

Post-Hospitalization Recovery Support – Assistance with transitioning back to daily routines following surgery, illness, injury, or hospitalization.

Mobility and Safety Support – Coordination of therapy recommendations related to transfers, assistive devices, and fall-risk reduction.

Individualized Therapy Goals – Collaboration with licensed therapists to support each resident’s abilities, preferences, and care objectives.

Care Team Coordination – Communication with physicians, therapists, families, and caregivers to promote continuity of care and follow recommended treatment plans.

Progress Monitoring – Observation of functional changes and communication of significant updates to the appropriate healthcare providers.

Rehabilitation therapy services are provided by qualified licensed therapy professionals in coordination with the resident’s healthcare provider and according to the resident’s individual needs and care plan.
